(CNN) -- Chelsea striker Andriy Shevchenko is to rejoin his former club AC Milan after two largely disappointing seasons at Stamford Bridge.
Shevchenko rarely showed the form which attracted the attention of Chelsea.
The deal will go through on Monday, subject to a medical, but neither club would confirm if the move was permanent or a loan deal.
"Chelsea have agreed terms for Andriy Shevchenko to return to AC Milan," said a statement on the Chelsea Web site. "Details of the agreement will remain confidential."
Ukrainian international Shevchenko joined Chelsea in the summer of 2006 for an English record £31 million ($62 million) but has scored just nine league goals in two seasons, as injuries and lack of sharpness belied his reputation as Europe's leading striker.
It was in sharp contrast to his scoring record in Serie A with Milan, notching 127 goals from 208 games.
The former European champons are delighted to have acquired his services again as club vice-president Adriano Galliani explained to their television channel.
"I believe that Milan are the only club in the world to have three European players of the year in our squad." Galliani said.
"Andriy has returned home and I am happy about it. I am a Milan fan and I am very happy," he added.
Shevchenko began his career at Dynamo Kiev, winning five league titles and two national cups with the team between 1994-99.
His much-vaunted arrival at Chelsea was reported to have increased the rift between then manager Jose Mourinho and the club's billionaire owner Roman Abramovich.
His successor Avram Grant used Shevchenko only sparingly and new boss Luiz-Felipe Scolari has now deemed him surplus to requirements.
